Why are there two separate forms?
The Optimization Assessment captures your goals, symptoms, priorities, and desired outcomes. The Client Consent Form records the required consent and securely connects your contact information to the review pathway. Completing one does not complete the other.
Which form should I complete first?
Start with the Optimization Assessment, then complete the separate Client Consent Form. Doing them in order gives the team the clearest starting point.
Do I need a BalanceTest before the review?
The review pathway is designed around your BalanceTest results. If you have not completed a test or are unsure what is ready, submit both forms and the team can confirm the appropriate next step.
Can this be completed virtually?
Yes. The forms are online, the test is completed at home, and the educational results review can be conducted by video, making the pathway available beyond the local Arizona office.
Is this a medical or peptide consultation?
No. The BalanceTest Results Review is educational and explains your test results and cellular nutrition foundation. It is not medical advice, diagnosis, treatment, or a peptide consultation.
What if I already completed one form?
Complete only the missing step. If you are uncertain which form was submitted, contact the PCW team rather than sending duplicate entries.
