Your choices
For certain health information, you may tell us your preferences about sharing information with family, friends, or others involved in your care; disaster-relief situations; fundraising communications; and other uses requiring authorization. We will obtain written authorization where required, including for uses not otherwise permitted by law.
How we may use and disclose information
